Dear Parents, Prospect Kids, LLC. would like to offer you the opportunity to give us feedback about the services our Professionals provide to your children. We take every effort to assure the highest quality of services is delivered through our providers and administration to your child and family. It is very important for us to be aware of your opinion, suggestions and comments on our work. |WHO IS ELIGIBLE?|
To be eligible for Early Intervention Services, your child
must • be 0-3 years of age • have a developmental delay
|have a diagnosed physical or mental condition that has a high probability of resulting in a delay Early Intervention is a public program funded and regulated by New York State Department of Health, and in NYC by the Department of Health and Mental Hygiene. All services are provided at no out-of pocket cost to families, but health insurance may be accessed for reimbursement of these services. Prospect Kids serves children in Brooklyn, Queens, Manhattan, Bronx, & Staten Island.|
|If you suspect that your child may be delayed in one or more area, or if your physician expresses concerns, a referral can be made directly for the Early Intervention Program by calling 311 or contacting a State Approved Agency.|
|SHOULD I BE CONCERNED?|
Is your child:
• not rolling, walking, or moving like other children the same age?
• not talking or talking differently than other children?
• having problems seeing or hearing?
• having health problems?
• too active or not active enough?
If you answered yes to any of these questions, you may want to have your child evaluated under the Early Intervention Program.
|The earlier a developmental concern is identified, the better. With early detection, a child can receive help in specific areas while simultaneously maintaining progress in other areas of development. Families, important teachers for their children, work closely with their service coordinator and the evaluator to develop and implement an Individual Family Service Plan. Program areas include special instruction; speech, physical, and occupational therapy; service coordination; and other services as outlined in their plan. CORE: A multidisciplinary evaluation to assess all areas of development. Supplemental: May include speech, physical, and/or occupational therapists.|
